To better assist patients who do not currently have dental insurance, we offer the following In-Office Benefits Program
Our in-office benefits program provides comprehensive and professional dental care to our patients without claim forms, pre-authorizations or deductibles. Patients pay a yearly fee and receive the following services at no additional cost:
- All Digital X-rays taken during initial evaluation
- Comprehensive Evaluation
- Periodic Evaluation
- Adult Cleaning
- Adult Fluoride Treatment
- Child Cleaning
- Child Fluoride Treatment
The following services will be discounted at 50% off the cutomary fees as long as they are completed within the same year:
- 6 Month – Adult Cleaning (2nd cleaning of the year)
- 6 Month – Adult Fluoride
- 6 Month – Child Cleaning
- 6 Month – Digital X-rays taken during 6 month evaluation
- All other procedures will be discounted at 15% off the customary fees.
The cost to the subscriber of our Benefits Program is as follows:
- Single Member Yearly Fee $225.00
- Two Member Family Yearly Fee $325.00
- Three Member Famly Yearly Fee $400.00
- Each Additional Family Member $75.00
